Due to the constant changes in health care coverage, it is the patient’s responsibility to determine what is covered by their insurance plan before seeking treatment. This includes all referrals and co-pays.
We request 24-hour notice in case of a cancellation or to reschedule. If you do not attend a scheduled appointment, the doctors reserve the right to terminate care and/or charge for the missed appointment. Weather and emergency situations are taken into consideration.

Prescriptions (unless written or changed) MUST be called into your pharmacy so we will have all the information necessary to authorize refills. Patients need to be seen at least every 3-6 months to renew medications. Weekend refills of prescriptions are NOT AVAILABLE.
If medications are not taken as prescribed, or if a patient is getting medications from other physicians without Dr. Hall’s approval, we reserve the right to terminate care.
